Community Case Management Services (CCMS)
Community Case Management Service (CCMS) is a service provided by professional case managers who have a background in social work or nursing. Community Case Managers are trained to assess the needs of the elderly and to find the appropriate services to support them and their caregivers.
The main objectives of CCMS are to:
1. Help the elderly (individuals aged 60 years and above) remain active in the community
2. Provide support to caregivers
3. Prevent premature institutionalization of the elderly. If an elderly person has multiple medical, physical and social needs and requires community support, s/he will benefit from CCMS.
