This programme provides needy, ill or bedbound patients who have just been discharged from hospital with a range of services comprising home modifications, transport for follow-up care, medical equipment, and supplies such as diapers or dressings.

CareLink provides discharge planning services for patients and their caregivers. It assesses the patient's post-discharge needs based on his medical condition, financial situation, and availability of a caregiver at home; and refers him to a suitable vendor / service provider. Click

This department assists patients and families in coping with psycho-emotional, social, environmental, and practical issues arising from the medical conditions faced, towards facilitating the recovery process and enabling patients (and their cargivers) to reintegrate into their homes and community.
