The Assessment & Coordination for Enabling (ACE) programme is a pilot programme by the Centre for Enabled Living (CEL).
ACE assesses, matches, and coordinates multiple social care and support services to the needs of seniors needing care. The programme assists families in managing the care they provide to their ageing loved ones by providing community-based support. This enables them to live with their family members and within the community for as long as possible, and avoid/minimise premature admission into institutions such as nursing homes.
Who
Seniors aged 60 and above who are Singapore Citizens or Permanent Residents, and require two or more community-based services
(Persons needing care who are <60yrs will be considered on a case-by-case basis.)
How
Speak to a social worker or medical social worker for assistance in submitting the
referral form to CEL, or directly contact an ACE consultant from CEL.
Fees
There is a one-time nominal fee (based on type of accommodation) for the services provided. This fee is not paid to CEL, but to the ACE service partners.
For Singapore Citizens
Private property: $100
HDB 4-room to Executive: $50
HDB 3-room and below (including rental flats): $10
For Permanent Residents*
Private property: $150
HDB 4-room to Executive: $75
HDB 3-room and below (including rental flats): $15
*There will be no waiver of nominal charges for Permanent Residents.
